TERMS & CONDITIONS OF USE
1.Users of the Community Transport vehicle must read and agree to be bound by the following conditions of use. 2.The users must be a community or voluntary organisation operating within the SRB 5 area. 3.Each application is subject to approval and the availability of the vehicle. 4.The named driver/s must produce their driving licence and one other form of identification before the vehicle keys are handed over. 5.If driving is carried out by an unlicensed driver or unauthorised driver any liability which may arise out of an accident will fall on the organisation/person using the vehicle and not the Kinsley and Fitzwilliam Community Resource Centre. 6.The driver must carry out a vehicle check before using the vehicle and also on its' return and complete the 'vehicle condition form.' 7.The vehicle must be returned in a clean and tidy condition and fuel used must be replaced. 8. No liability for an accident must be accepted or implied when exchanging those particulars required by law. The other party involved should be referred to the Kinsley and Fitzwilliam Community Resource Centre Manager. 9.Accidents involving the vehicle must be reported to the Kinsley and Fitzwilliam Community Resource Centre Manager at the earliest opportunity. 10.No person shall use any special equipment that is fitted to the vehicle, unless they have received full instructions on their correct use. 11.Wheelchair passengers must be secured in the vehicle with at least two of the special clamps provided. The passenger must also be secured into their wheelchair by means of an independent seat belt/harness. Belts fitted to the wheelchair are not acceptable as they are for use outside the vehicle. 12.Please do not smoke in this vehicle. 13.When returning the vehicle outside normal office hours, the driver should ensure that the vehicle is left in the parking bay and that the compound gates are locked. The bus and gate keys should be returned to Reception the next working day.
IN CASE OF EMERGENCY---- PLEASE READ CAREFULLY
THE VEHICLE MUST NOT BE USED IN AN UNROADWORTHY CONDITION. THE CHECKS ON THE VEHICLE CONDITION FORM ARE REQUIRED BY LAW AND MUST BE CARRIED OUT BEFORE EACH JOURNEY. ANY DEFECTS SHOULD BE REPORTED IMMEDIATELY EVEN IF IT MEANS THE JOURNEY MUST BE ABANDONED.
ˇShould a breakdown/accident occur whilst on a journey telephone the contracted breakdown service, AA 0800 374457(card in bus)who will assist you. During office hours you should also inform the Kinsley and Fitzwilliam Community Resource Centre on 01977 610931. The community centre will use the passenger list to contact and inform relatives of any breakdown/accident/delay.
ˇA copy of the passenger list should be kept on the vehicle at all times to assist the emergency services in case of accident.
ˇShould a breakdown/accident occur out of normal office hours telephone the contracted breakdown service. If a severe accident occurs with injuries to passengers, in addition to contacting the emergency services please contact the following out of hours telephone numbers.

A report on the circumstances of the breakdown/accident must be submitted as soon as possible to the Kinsley and Fitzwilliam Community Resource Centre Manager. Where an accident has occurred, an Accident Report form (available on request) must be completed for insurance purposes.
